ASSUMPTION OF RISK & RELEASE OF LIABILITY

By participating in the South Central Championship box lacrosse event (the “Event”), I understand and acknowledge that box lacrosse is a fast-paced, high-contact sport that involves inherent risks of serious injury. These risks include, but are not limited to, collisions, falls, physical contact with other participants, being struck by the ball or equipment, facility hazards, and other unforeseen circumstances that may result in injury, disability, or death.

I voluntarily choose to participate in the Event and assume all risks, known and unknown, associated with participation.

In consideration for being permitted to take part in the Event, I hereby release, discharge, and hold harmless Super G Lacrosse Inc., its owners, directors, officers, employees, contractors, volunteers, event staff, and facility partners from any and all liability, claims, demands, or causes of action arising from any injury, loss, or damage that I may sustain while participating, whether caused by the negligence of Super G Lacrosse Inc. or otherwise.

I understand that I am solely responsible for any medical expenses or treatment required as a result of participation, and I authorize Event staff or medical personnel to provide or arrange emergency care if necessary.

By signing this waiver, I confirm that I have read and understand the above statements, accept all associated risks, and agree to be legally bound by this release. Participants under 18 must have a parent or legal guardian agree to these terms on their behalf.
